Please have the following forms and information with you when you arrive for your initial appointment:

• Massage Therapy Health History Form - NEW

• Pediatric Client Intake Form - NEW

• Follow-Up Care for Deep Tissue Massage - New

• Insurance Card

• Prescription/Referral doctor can fax referral to 360.623.1215

No-call/No-show Policy

• First no-call/no-show: 50% cash value of service.

• Second no-call/no-show: 75% cash value of service.

• Third no-call/no show: total value of massage and subsequent massages must be paid in advance
